Take steps to improve your health and well-being with these health and wellness resources:

UniCare primary nurses – You can work with a UniCare primary nurse at no extra charge to get one-on-one support for a health condition or help with reaching your health goals. These nurses are dedicated to your personal care and to helping you and your family members enjoy your best health. (For Basic, PLUS and Community Choice members only.)

24/7 NurseLine – When you have concerns about your health, call our 24/7 NurseLine at 800-424-8814. The NurseLine's registered nurses are available around the clock to answer your questions or listen to your health concerns. Important! Be sure to select the menu option for the NurseLine.

Employee Assistance Program (EAP) resources (for active state municipal employees and their dependents) Check out the wide variety of free resources available to you through the Mass4YOU EAP. You can also view a short video about your EAP benefits.

Health assessment – Take the health assessment to find out how healthy you are and get customized suggestions to improve your health. To access the health assessment, you will first need to log into the secure member portal at unicare.com.

Preventive care is an important key to your health. Find out which screenings and other services are covered based on your age, sex and health. These services are covered at 100%, so don't miss out on these important ways to protect your health.

Not sure which plan you're in? You'll find your plan name on the front of your ID card, under your member ID number.

Discounts on health-related items – Our SpecialOffers program offers you discounts on many health-related products and services, like health club fees for new memberships, programs for weight management and stress reduction, discounts on baby items and more.

Important! You can only access SpecialOffers discounts from the unicare.com member portal. There, you'll find a complete list of all the discounts available to you.

Fitness club reimbursement – You can get reimbursed for up to $100 toward membership at a fitness club. The reimbursement is paid to the Plan enrollee upon proof of payment. Use our fitness club reimbursement form to request your reimbursement. Check the instructions for details on the types of facilities that qualify under this benefit and how to complete the reimbursement form.

Diabetes Prevention Program Reimbursement Form (for non-Medicare members only) — You can get reimbursed up to $500 when you complete 20 or more sessions in an approved diabetes prevention program. See the reimbursement form for details.

Tobacco cessation

  • Counseling for tobacco dependence/smoking cessation is covered up to a limit of 300 minutes per plan year. It is reimbursed up to the Plan's allowed amount.
    • Counseling can occur in a face-to-face setting or over the telephone, either individually or in a group.
    • Counseling may be provided by physicians, nurse practitioners, physician assistants, nurse-midwives, registered nurses and tobacco cessation counselors.
    • For information about additional counseling at no cost, call UniCare Member Services toll free (833-663-4176 for Basic, PLUS and Community Choice members and 800-442-9300 for Medicare Extension members).
  • Tobacco cessation counseling can be billed directly to UniCare by the provider. However, if your provider is unable to bill the Plan directly or does not accept insurance, you may submit a claim form to be reimbursed for tobacco cessation counseling. Check the instructions for more information about coverage and reimbursement.
  • Nicotine replacement products are available at no cost through your prescription drug plan if you have a prescription. See your member handbook for details.

So if you're familiar with your condition and ask good questions, you can do a lot to ensure safer care.
