Enrollee Assistance Program (EAP) resources (for Basic, PLUS and Community Choice members) – You have access to a wide variety of free EAP resources through Optum.
Health assessment – Take the health assessment to find out how healthy you are and get customized suggestions to improve your health. To access the health assessment, you will first need to log into UniCare's secure member portal. Then choose “Health and Wellness.”
Fitness club reimbursement – You can get reimbursed for up to $100 per family on your membership at a fitness club. The reimbursement is paid once per plan year to the Plan enrollee, upon proof of membership and payment. Use our fitness club reimbursement form to request your reimbursement. Check the instructions for details on the types of facilities that qualify under this benefit and how to complete the reimbursement form.
Diabetes Prevention Program Reimbursement Form (for non-Medicare members only) – You can get reimbursed up to $500 when you complete 20 or more sessions in an approved diabetes prevention program. See the reimbursement form for details.
Wellness topics – Explore ways to improve your health and well-being with videos, presentations and other materials covering a broad range of important topics.
eHouseCall newsletters – Read up on healthy living tips for the whole family. Each monthly issue features a health condition or preventive step, plus tips and interesting facts.
Patient safety – Being an educated patient can help prevent medical errors. One of the most frequent factors in medical errors is poor communication. So if you’re familiar with your condition and ask good questions, you can do a lot to ensure safer care. Get helpful tips on preparing for office visits and learn what questions you should ask your doctor and other health care professionals to help ensure that you get safe care. Then select Health and Wellness and choose the link for patient safety information on the right side of the screen (under Useful Tools).
Preventive Care Guidelines – Preventive care is an important key to your health. Find out which screenings and other services are covered based on your age, sex and health. These services are covered at 100%, so don’t miss out on these important ways to protect your health. Not sure which plan you’re in? You’ll find your plan name on the front of your ID card, under your member ID number.
