Collaborative Learning Opportunities for the Patient-Centered Primary Care program make up a multi-faceted practice transformation curriculum that collectively ensures our practices have access to the country’s leading health care experts, the most current information, and reliable resources – all in support of practice transformation and fulfilling Patient-Centered Primary Care program expectations.

To help you satisfy your continuing education requirements, our Collaborative Learning curriculum offers the potential to earn as many as 30 continuing education credits through live events or on-demand recordings. The Patient-Centered Primary Care team works hard to offer you learning opportunities designed to help you succeed.

Upcoming National Collaborative Learning Webinars – These virtual sessions are an opportunity to hear from national experts in health care who can offer real-world solutions and guidance around practice transformation, evidence-based medicine and patient-centered care. Each of these learning opportunities is carefully designed to offer relevant content to practices at all stages of adopting patient-centered care, and to support practices’ fulfillment of Patient-Centered Primary Care program obligations.

Guidance that addresses the Triple Aim (a better patient experience, lower cost of care, and improved outcomes) is woven through every session.

Pediatric learning collaborative sessions are a subset of national events. These events are created via an ongoing partnership with the American Academy of Pediatrics, in recognition of the specific challenges that come with caring for younger patients.

Collaborative Learning Recording Library – The Collaborative Learning Recording Library houses recordings of prior National Collaborative Learning webinars that address some of the most pressing topics in health care today, as well as on-demand presentations developed specifically for self-guided development. All are available for immediate viewing.

Practice Essentials – The Practice Essentials curriculum is a set of virtual sessions designed to help primary care practices take the first steps in practice transformation and move along the medical home continuum. This curriculum was developed specifically for primary care and offers continuing education credits.

Through this comprehensive, customizable curriculum, we guide practices step-by-step through quality improvement methodology to achieve sustainable change and improve patient satisfaction, clinical outcomes and value. This virtual curriculum is offered on multiple platforms (live, recorded, and face-to-face) to meet the needs of your practice and accommodate your style of learning.

Achieving the Triple Aim Learning Opportunities – CEU/CME credits available for your transformation education. UniCare’s education series, Achieving the Triple Aim, bundles together interventions to tackle cost, quality, and patient experience. These intervention bundles are generally three to five small, straightforward sets of evidence-based practices that, when performed collectively and reliably, have been proven to improve patient outcomes.
