ID Cards and Updated Benefits Information for Renewing Members for July 1, 2009

Thank you for continuing your membership with the UniCare State Indemnity Plan. We look forward to continuing to serve your health plan needs.

New ID Cards

New ID card(s) were mailed to you during the third week of June. If you do not receive your card(s) by July 1, please contact us toll free at (877) 633-6396. In the meantime, you can continue to use your current ID card.

However, please note that if you are a Basic, PLUS or Community Choice member, the copays on your current ID card do not reflect copay changes that are effective July 1, 2009. Your replacement card shows your new office visit and emergency room copays. It also adds copay information for visits to retail medical clinics. Medicare Extension (OME) members: please note that you have no copay changes for the new Plan year.

Updated Benefits Information

You will also receive the following Plan information, which will be mailed to you separately from your ID cards(s) during the first week of July.
  • PLUS Plan Members - You will receive a new member handbook that reflects your coverage effective July 1, 2009.


  • Basic and Community Choice Plan Members - You will receive a benefit update that shows how your coverage has changed for the new Plan year and includes other important plan information.
