ID Cards and Welcome Package Materials for Members Enrolling Effective July 1, 2009

Your ID card(s) were mailed to you during the third week of June. If you do not receive your card(s) by July 1, please contact us toll free at (877) 633-6396. Your new plan coverage begins on July 1, 2009. If you need to receive medical services before you receive your new Plan ID card(s), your provider can contact UniCare Customer Service at (800) 442-9300 to verify your eligibility.

You will also receive a welcome package, which will be mailed to you separately from your ID card(s) during the first week of July. This package will include your member handbook and other information to help you become familiar with your new health plan.
